Retain Leonard Law Group
You can retain Leonard Law Group for your workers compensation claim by completing the workers compensation questionnaire and signing and dating the Application for Adjustment of Claim, Attorney Representation Agreement, Workers Compensation Retainer and two HIPPA Medical Authorizations. You need not fill out the Application, Attorney Representation, Retainer and HIPPA Medical Authorizations, only sign and date each where the “x” indicates. Do your best to fill out the questionnaire. If there is information you do not know don’t worry.

Once you have mailed, faxed, emailed, dropped off or scanned the questionnaire and other legal documents to Leonard Law Group our representation of your claim begins. Our mailing address is 300 S. The Application and Attorney Representation will be filed on your behalf at the Illinois workers compensation commission and you will be assigned a case number. These filed legal documents will be forwarded to you within fourteen business days. Please retain them for your records.
The workers compensation retainer spells out the terms of our legal relationship. You will agree to pay Leonard Law Group 20% of your total settlement or trial award as well as reimburse Leonard Law Group for all expenses associated with the prosecution of your claim. The remaining 80% is the clients tax free. You pay no up-front retainer fee and if we do not recover for you no fee is owed. We have a 99% success rate representing injured workers in all types of trades.
The final two legal documents are the HIPPA Authorizations. By executing these you allow Leonard Law Group to issue subpoenas for your medical records. That is the sole purpose of the HIPPA Authorizations.
Finally, after you have forwarded the questionnaire and legal documents to Leonard Law Group our staff will call you to confirm receipt and inquire which partner you wish to represent you. Please note if you already have hired an attorney please notify our staff at that time so we can ensure a smooth transition from his/her office to Leonard Law Group.
